COORDINATION OF SUPPORTS – LEVEL 2 SUPPORT

Coordination of Supports is a standard level of support coordination that is approved in majority of NDIS plans. We will assist you to build the skills you need to understand and implement supports to get the most out of your Plan.

Our team understands how challenging it can be to navigate the NDIS, decipher plans or speak with service providers to ensure you are gaining the most from your services.

We work alongside you, and together, develop a solutions-based approach to tackle the challenges you may be facing as well as ensuring that you feel supported along the journey.

Our support coordination team will assist you to understand and implement the funded supports in your plan and link you to community, mainstream and other government services. We will help build your ability to exercise choice and control, to coordinate supports and access your local community.

Our Support Coordination Team can help with the following

Assist you to understand the NDIS; what it means for you and how can you use the supports in your plan to have a good life.

Help you navigate the NDIS system.

Provide awareness of what “choice and control” means and how providers can assist in the delivery of NDIS services.

To assist you to develop capacity to tackle situations independently.

Provide assistance to connect with providers, troubleshoot issues that may arise and provide advocacy when working with providers.

Support to link to community and mainstream supports.

Understand what you want from the NDIS and how the NDIA can best support you.

Assist to explore housing options.

Assist to explore community and economic participation.

Assist to implement nursing and therapeutic supports.

Plan reassessments and change of situation processes.

Transition from secondary school.

Coordination of unexpected crisis situations.

Shine Support Coordination and Consultancy Services promotes a matching process as part of the initial referral screening and assessment process. The identification of a matching process ensures that the worker with the relevant experience is able to facilitate supports. Our support coordinators work in small teams and are able to share knowledge of evidence based approaches to assist you to achieve identified goals.
