TRANSITIONAL SUPPORT PLANNING
Transitional Support Planning is there to assist you when you find yourself in a key-life transitional period, and need some additional support to navigate key life stage transitions.
This could mean helping you with your search for employment after finishing school, transitioning you from a rehabilitation facility, or hospital, or even helping you to move out of home for the first time.
Transition support planning can be accessed from the capacity building budget and supports can include some of the following:
WHAT IS PERSON-CENTRED PLANNING
Person-Centred Planning forms part of a toolkit that is used to bring together various information and people. It involves talking with you and your informal and formal support networks, and working with you to find practical solutions for success in achieving your NDIS goals. All person-centred planning approaches are built on a qualitative methodology and that achievement is driven from a series of discussions and evidence-based practices.
What is Independent Living Option Exploration?
An Option Independent Living Option is a support that is funded by the NDIA that lets you have control over who supports you in your home and how supports will be provided. Independent Living Option Exploration, is where the team at Shine Support Coordination & Consultancy Services (Shine SCCS) investigate which option would be best suited to you and your needs, and run through what the eligibility criteria looks like. It could mean that you would be able to have someone live with you in your own home or you could move in with someone on a temporary or ongoing basis.
